Attendance Card
For 2019, we will, once again, be using the following attendance card for all athletes. All athletes will be given a hard copy of this on August 2, 2019.
2018 - 8th Grade Open House Letter
If you missed 8th grade open house, but are interested in playing volleyball in the fall of 2019, please down the attached information.
Athlete and Parent Information Sheet
All prospective athletes need to complete and submit this paperwork to the Northview Coaching Staff.
Daily Checklist for Athletes
See the attached link for a checklist for athletes for both match days and practice days. All equipment must be brought each day.
OHSAA Pre-Participation Physical Form
All athletes must have a valid OHSAA Pre-Participation Physical Exam prior to participation in Volleyball. Athlete's must see a physician and have this returned to the Volleyball Coaching Staff prior to participation.
